Geriatric Evaluation
and Management Services
Geriatric
Assessment includes:
- Unexplained or
repetitive injuries (falls, burns, bruises, cuts)
- Unsteady walking or
balance
- Swallowing
difficulty, choking on foods or fluids
- Consistent,
unplanned and unexplained weight loss
- Difficulty managing
medications appropriately
- Increasingly unsafe
accumulation of paper or clutter in the home environment
- Unmanaged
incontinence
- Becoming lost,
wandering
- Lack of maintaining
usual weekly social routine
- Increasing
forgetfulness, mental confusion, or repetitive statements
- Failure to cook or
eat
- Reluctance to
invite people in the home
- Change in
personality
In
order to design a problem-oriented plan for the future care to include:
- Treatment and
therapy planning (medical, psychiatric, PT, OT)
- Rehabilitation
planning
- Long term placement
planning
- Coordination of
health care provider goals (resolve medication and/or health care provider
recommendation conflicts)
- Optimized use of
available resources
The
overall goals of a Comprehensive Geriatric Assessment are to:
- Improve diagnostic
accuracy
- Guide the selection
of interventions to restore or preserve health
- Recommend an
optimal environment for care
- Predict outcomes
- Monitor clinical
change over time
……and most importantly, improve quality of
care and maintain independence
