resource materials provided to patient and family
- A written summary report
is promptly forwarded to the primary care physician
- Referrals to
specialized physician services, community resources, geriatric case
managers, legal services, homecare services or senior agencies
- Personalized short
term and long term care planning
- Assistance for
proper placement in various settings – private home, group home, assisted
living and nursing home
Dr.
Lessig is now accepting new patients who are:
- Home-bound
individuals that are unable to visit the Doctor’s office
- Individuals who
would require specialized transport due to disability or pain
- Residents in
assisted living or group homes
- Enrolled in home
hospice care
- Suffering
Alzheimer’s disease or dementia related conditions with or without
behavioral disturbance
